      a copy of the document confirming the right of use of the applicant for the facility mentioned in the application;

    • The name of the document :

      a copy of the contract with the relevant enterprise and organization for conducting disinsection and deratization measures against insects and rodents in hotels and hotel-type facilities;

      Document submission forms :

    • The name of the document :

      contract concluded with the relevant enterprise or organization on the transportation of household waste.
