A citizen of the Republic of Azerbaijan :

    • The name of the document :

      Document of higher and secondary education (in case of a document of higher education in a foreign country, including a certificate of admission to medical and pharmaceutical activities (since 1999)

      Document submission forms :

      - Copy of the document (paper)

    • The name of the document :

      ID card

      Document submission forms :

      - Information is entered into the electronic system

    • The name of the document :

      Document of transfer to residency training (diploma), certificate of internship, and document of permanent appointment (if available)

      Document submission forms :

      - Copy of the document (paper)

    • The name of the document :

      Document submission forms :

      - Copy of the document (paper)

    • The name of the document :

      Work record book (with the exception of those who have started to work for the first time)
