Content

- Registration of change of name, patronymic and surname. Citizens of the Republic of Azerbaijan may change their name, patronymic, and surname at the age of 18. If the applicant so wishes, they may adopt the patronymic of the person who brought them up.

Documents required for implementation and their submission form

A citizen of the Republic of Azerbaijan :

    • The name of the document :

      Notarized copy of the document confirming the identity of the applicant

      Marriage certificate, if the applicant is married
